Service Description
Community Aged Care Packages provide an at home care alternative for people whose dependency and complex care needs would normally qualify them for entry into a low care residential facility.
A Community Aged Care Package is an individually tailored package of care that can include personal and/or domestic assistance and wellness and lifestyle services.
Care Package
Our care package may include:
- comprehensive care planning and ongoing assessments
- Domestic Care
- Personal Care
- Wellness and Lifestyle
- Respite Care
- Life Link Personal Alarms and Monitoring
- Life Link Telecare Monitoruing (emergency response)

Signature Features
Our signature features that can ‘make a difference’ include:
- services delivered by professional and qualified care-givers who have a passion for working with aged people and people with a disability and see it as a privilege to do so
- use of best practice guidelines to ensure services are targeted and responsive to individual needs
- wellness and lifestyle oriented activities that promote socialisation and healthy ageing
- manicures, hand massages and companionship provided by our friendly volunteers
- counselling and chaplain services
- transport to and from planned Feros Care activities and outings
- assistance with arranging regular and planned respite breaks
- priority access to other Feros Care community, respite and residential care services
Eligibility
All applicants for Community Aged Care Package services need to be assessed by an Aged Care Assessment Team (ACAT).
ACATs help older people and their carers work out what kind of care will best meet their needs when they are no longer able to manage at home without assistance. Click here for more information on the role of ACATs.

Feros Care considers the ACAT assessment in consultation with prospective residents, their carer(s), significant others and health providers in developing individualised care plans.

Think about what support you need to continue living at home. What daily living tasks are becoming difficult? Discuss your needs with family and friends to get their opinion. |
RING |
Ring our Aged Care Advisory Team on 1300 763 583. You will be allocated a Feros Care Manager who will help you undertsand your eligibility requirements and refer you to your local ACAT for a mandatory assessment. Arrangements can also be made to visit the day centre to try out the service. |
DECIDE |
Our Feros Care Manager will answer any questions you may have about the service including fees and care planning. Discuss your decision with family and friends. Review the care plan and sign the Care-Recipient Agreement which outlines the terms and conditions of service delivery. |
BOOK |
Book the type and frequency of service you need with your Feros Care Manager. |
RECEIVE |
Prepare to receive the care you need to stay at home and look forward to an improved quality of life. |
Value Adding
The following services may be available upon request for an additional fee:
- some transport outside designated shires
- some planned activities and outings
Affordability
Community Aged Care Packages are subsidised by the Commonwealth Government.
Care-recipients are asked to pay a fortnightly fee.
For seniors on the maximum basic rate of pension, fees must not exceed 17.5% of that pension.
Seniors on higher incomes may be asked to pay additional fees limited to 50% of any income above the maximum pension rate.
Allowances and subsidies are available for those families who are financially or socially disadvantaged.
A direct debit arrangement where fees are deducted fortnightly from a nominated bank account is the preferred method of payment. Feros Care also accepts credit card, cheques or cash payments (at any Feros office).
