Service Description

Home and Community Care’s Physiotherapy services provide exercise and physical therapy for seniors and those with a disability to help improve balance, strength and stamina.  The program consists of gentle exercises that suit all levels of ability.  Home and Community Care programs offer a basic range of social, wellbeing and support mechanisms to assist people who would otherwise be prematurely or inappropriately admitted into residential care, to live independently in their home within the community.

Home and Community Care physiotherapy, personal and domestic care services can be used in a combined care package depending on regional availability.

Care Package

Our care package may include:

  • comprehensive care planning and ongoing assessments
  • provision of therapy services focussed on mobility and movement.
  • maintaining independence program conducted in Day Centres throughout the Far North Coast of NSW.
  • Maintaining independence program At home – an individual home mobility and exercise program.
  • Individual assessment and treatment programs for eligible clients

Eligibility

To be eligible for a Home and Community Care service the care-recipient must:

  • be living at home
  • be an older and frail person or person with a disability
  • have difficultly doing everyday tasks
  • be a carer of a frail older person or person with a disability
  • be likely to need to go into an aged care home or hospital if not being provided with Home and Community Care services
  • not already be in receipt of other Government-subsidised services that are similar to Home and Community Care services

Home and Community Care recipients do not need to be eligible for residential care to receive services.

Eligibility is assessed by a Feros Care Manager in consultation with the propsective care-recipient, their carer(s), significant others and health professionals. This information is used to develop an individualised care plan.

The Commonwealth Carelink Centre provides free and confidential information on community aged care, disability and other support services that can asist in determining eligiblity for Home and Community Care. Contact the Commonwealth Carelink Centre on 1800 052 222.

THINK

Think about what support you need to continuing living at home. What daily living tasks are becoming difficult? Discuss your needs with family and friends to get their opinion.

RING

Ring our Aged Care Advisory Team on 1300 763 583. You will be allocated a Feros Care Manager who will help you understand your eligibility requirements and refer you to your local ACAT for a mandatory assessment.

DECIDE

Our Feros Care Manager will answer any questions you may have about the service including fees and care planning. Discuss your decision with family and friends. Review the care plan and sign the Care-Recipient Agreement which outlines the terms and conditions of service delivery.

BOOK

Book the type and frequency of service you need with your Feros Care Manager.

RECEIVE

Prepare to receive the care you need to stay at home and look forward to an improved quality of life.

Value Adding

The following services may be available upon request for an additional fee:

  • some transport outside designated shires
  • some planned activities and outings
  • Life Link personal alarms and Telecare monitoring

Affordability

The Home and Community Care Program is jointly funded by the Commonwealth, State and Territory Governments. In New South Wales, the program is administered by the Department of Ageing, Disability and Home Care.

Fees are subsided by the Home and Community Care Program.

The basic daily fee for individual assessments and therapy sessions is approximately $10 per hour for direct care and may vary depending on assess need and financial capacity.. Group Therapy sessions for HACC funded day centres are approximately $50 per session.

Allowances and subsidies are available for those families who are financially or socially disadvantaged.

A direct debit arrangement where fees are deducted monthly from a nominated bank account is the preferred method of payment. Feros Care also accepts credit card, cheques or cash payments (at any Feros office).
