Service Description
Feros Care’s Telehealth Monitoring Solutions provides support to those living with single or multiple long term conditions, including Chronic Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), diabetes, and hypertension, to help them continue living independently and better understand their health care, therefore improving their health and lifestyle.
The Telehealth monitors are simple to use, and allows people to monitor their vital signs daily using common devices such as weight scales, blood pressure monitors, blood glucose monitors, and peak flow meters. The daily readings are taken by the client and the information is automatically sent through to Feros Care’s Life Link team. This data can be accessed securely by clinical health professionals to make informed decisions about the clients health and well-being.
