Service Description

Palliative care is end of life care given to improve the quality of life for individuals who have a serious or life-limiting disease. Its goal is to alleviate the symptoms, treat side effects, and provide or facilitate psychological, social and spiritual support.

Our palliative care team helps those in the early stages of palliation or with non-complex treatment programs to palliate in the comfort of their home or residential facility.

Not everyone with a life-limiting disease needs or desires specialist care. Many non-complex palliative care needs can be appropriately met with the assistance of local GPs and Feros Care.

We aim to ensure a stress-free and well-supported transition from hospital to family or residential home by taking advantage of existing community resources and services.

All services are provided by highly qualified staff with an appreciation for the experience of palliation and are trained to respond to your needs and those of your loved-ones.

Care Package

Our care package includes:

  • comprehensive care planning and ongoing assessments
  • organisation and coordination of service delivery including Domestic Care, Personal Care, Nursing Care, Wellness and Lifestyle and Respite Care

Direct care services utilised under our palliative care program can be provided by Feros Care or an equally qualified and capable at home care provider.

Signature Features

Signature features available under this service may vary depending on the particular service programs accessed to meet care-recipient needs.

Eligibility

To be eligible for this service, care-recipients must have a palliative diagnosis.

Care-recipients can be referred to Feros Care by GPs, specialists or hospitals. Recipients can also self-refer to Feros Care by calling 1300 763 583.

Our experienced Palliative Care Manager will assess your needs in consultation with your carers, significant others and health professionals to develop an individualised care plan that utilises existing and recipient eligible at home care services.

Easy Access Steps

THINK

Think about what it would mean to palliate at home. What type of services would help you? Talk to your family and loved-ones about your ideas.

CONSULT

Consult your GP or specialist on whether or not your treatment program can be managed in a home setting.

TALK

Talk to our Palliative Care Manager who will assess your needs, explain what is involved in receiving at home services and develop an individualised care plan. Discuss this decision with family and friends.

SIGN

Review the care plan and sign a Care-Recipient Agreement which outlines the terms and conditions of service delivery.

RECEIVE

Prepare to receive the palliative care you need to stay at home.

Value Adding

The following services may be available upon request for an additional fee:

  • some transport outside designated shires
  • some planned activities and outings

Affordability

This service is subsidised by the Commonwealth Government under the National Palliative Care Program.

Service fees depend on programs accessed. In most circumstances a financial contribution of some sort is required.

Allowances and subsidies are available for those families who are financially or socially disadvantaged.

For Feros Care services, direct debit arrangement where fees are deducted monthly from a nominated bank account is the preferred method of payment. Feros Care also accepts credit card, cheques or cash payments (at any Feros office).
