Feros Care is excited to be recruiting for a Wellbeing Manager to join our team in the Grampians (ideally located within 1 hour drive of Ballarat). This is an exciting opportunity to be working with the Home Care Packages (HCP) and Short Term Restorative Care (STRC) programs, applying your case management skills to provide holistic person-centred approaches to care planning and coordination of services.
You will split your time between working from home and within the Grampians community to support your clients on their wellbeing journey. This is a full time role offered on a two year fixed term contract (with the possibility of extension based on funding).

About the role:
- Help your clients to remain at the center of their journey
- Develop care plans and coordinate a multi-disciplinary team (MDT) for the delivery of in-home therapy, services, and equipment
- Manage & Coordinate the day to day delivery of care for clients, including managing individualized budgets
- Advocate for the client and participate in case conferencing and scheduled clinical appointments as required
About you:
- Tertiary qualifications in Nursing or Allied Health (with relevant registration)
- Previous Case Management Experience (community and aged care experience highly desired)
- Experience working with clients conducting assessments and applying goal-oriented, outcome focused and holistic approaches
- Demonstrated ability to develop and maintain strong networks with internal and external stakeholders
- Current drivers licence and be comfortable traveling throughout the region each day
- COVID-19 Vaccination
