Case Manager - Lismore/Byron

On offer is a full-time home-based role with flexible hours that suit you and your clients. Work will be completed both virtually and within the community of Byron Bay / Lismore. An initial 12 month contract with view to extension is available.

The Role:

This is an exciting opportunity to be working with the Home Care Packages (HCP) program, applying your case management skills to provide holistic person-centred approaches to care planning and coordination of services to support wellness, reablement and independence.

The HCP program supports senior Australians with complex care needs to live independently in their own homes. You will be in control of managing your designated case load and supporting genuinely life changing outcomes for your clients.  As a Wellbeing Manager you will:

  • Help the clients to remain at the centre of their journey
  • Be responsible for managing individualised budgets, developing care plans, and coordinating the delivery of in-home therapy, services and equipment
  • Develop and maintain excellent relationships with key stakeholders

About you:

You will be motivated by providing exceptional client care, confident to work in a virtual environment, be a natural team player and have:

  • Case management experience – community and aged care experience highly desired
  • Experience working with clients applying goal-oriented, outcome focused holistic approaches
  • Excellent communication and customer service skills
  • Demonstrated ability to develop and maintain strong networks with internal and external stakeholders
  • Experience working remotely with a high degree of autonomy and drive
  • Current Senior First Aid and CPR qualifications
  • Current drivers licence
  • COVID19 Vaccination
