The Staying Healthy Living Well program is a FREE 12-week health literacy and education program to support your senior patients to improve their confidence, knowledge and engagement in managing their chronic conditions and general wellbeing.

Delivered by Feros Care and funded by Central and Eastern Sydney PHN, this innovative program has 3 key components to engage clients in self-managing their health condition/s and general wellbeing:

Remote monitoring of vital signs

Provision of simple to use touch-screen technology for patients to take regular vital signs readings. The readings are monitored by a Feros Care Telehealth Nurse who will provide education that links changes in vital signs to health behaviours, enabling improved understanding of patient actions and the effect on their risk factors.

1 on 1 coaching sessions

Individual sessions with Feros Care’s Telehealth Nurses to motivate patients to set goals to improve all aspects of their life – not just their health. Telehealth Nurses will assist them to access the services and community supports that will support them to stay active, engaged and connected.

Group education

Feros Care’s Virtual Senior Centre provides an opportunity for patients to meet others who are in similar circumstances. Our published evidence shows that video conference groups provide similar benefits to those experienced by face-to-face groups, such as social support and learning from each other.

The program takes a holistic approach to supporting patients in reaching their health and social care goals. It seeks to improve patient engagement in their health care through developing knowledge, skills and attitudes as well as facilitating access to services.

THE BENEFITS OF STAYING HEALTHY LIVING WELL

At the end of the 12-week program our goal is for patients to:

  • Have a better understanding of their chronic conditions and risk factors
  • Understand the lifestyle factors that may affect their health and general wellbeing
  • Feel more confident in managing their health conditions, with the ongoing support of their GP and healthcare professionals
  • Feel more confident with the use of technology
  • GPs can access vital sign trending reports for participating patients via My Health Record

How quickly can Feros Care start the program?

We will contact you as soon as we receive your referral. We will discuss with you and obtain parameters for vital signs readings, and any specific monitoring/coaching requirements for your patient. A Feros Care Wellbeing Manager will then visit the patient’s home, explain the program, configure the personalised vital ranges and daily wellness interview questions, creating a baseline assessment for vital sign ranges. They will also teach the patient to use the equipment.
