The End-of-Life Pathway is the government’s new program (starting from 1 November 2025) and it’s been thoughtfully designed to give you, or your loved one, the support needed to stay at home, surrounded by comfort, care, and familiar faces.

It’s about honouring wishes, supporting dignity, and making sure that every moment is lived with respect, choice, and a care team that walks quietly beside you. Never intrusive, but always there when needed. Because at Feros Care, we believe that every person’s final journey should feel truly theirs with comfort, compassion, and love.

So, what is the End-of-Life Pathway?

The End-of-Life Pathway offers extra support — up to $25,000 of funding over 3 months — to help you, or the person you love, stay at home when time matters most.

If you or your loved one has been told there may be less than three months to live, this funding helps make it possible to remain in the place that feels safest, most familiar, and most comforting — your own home.

It’s personalised, compassionate support, carefully matched to what’s needed, when it’s needed. No pressure. No rushing. Just thoughtful care that fits gently around your life, your wishes, and your family.

What kind of help could I get?

Every person’s wishes are different, and that’s exactly how the End-of-Life Pathway is designed: around you. The types of support available could include:

  • Nursing and palliative care to manage comfort, symptoms, and changing needs with skill and compassion.
  • Personal care to help you feel clean, comfortable, and cared for in a way that honours your dignity.
  • Emotional and social support — not just for you, but for your family too, because everyone’s wellbeing matters.
  • Essential equipment like beds, wheelchairs, or aids to make daily life simpler, safer, and less stressful.
  • Support for carers, offering a helping hand and a listening ear to those walking this journey alongside you.

It’s care that wraps softly around you and your loved ones offering reassurance, peace of mind, and the gentle support you need, without ever losing sight of what matters most: your wishes, your comfort, and your dignity.

How do I get started?

You don’t have to do this alone. We’re here to support you personally, with the respect you deserve. Or we can take more of a backseat if you would like some space. We’ll be guided by you and your wishes.

Step 1

Call our supportive Customer Care team first. We’ll listen, answer your questions, and help you understand your options.

Step 2

If you’d like to go ahead, we’ll help you connect with My Aged Care to organise an assessment with the government’s Single Assessment Team.

Step 3

Once you’re approved, we’ll work closely with you (and your family) to create a personalised End-of-Life Pathway plan quickly, respectfully, and always on your terms.

The End-of-Life Pathway is an aged care option when time matters most; therefore assessments and approvals are prioritised, and services are usually able to start within days, not weeks. This support is designed to help you live your final months your way, at home, with the people, the memories, and the comforts that matter most.
