Eat Well Live Well is designed to renew your interest in food and improve your appetite. As a tailored meal plan for older adults with a focus on easy meal preparation, Eat Well Live Well will give you the energy you need to live a full and active life.

Over six sessions, a qualified dietician will work with you to help set dietary goals and implement healthy eating changes – all designed to improve mealtimes at home.

To help you develop a healthy meal plan for seniors your age, we even take a peek into your pantry and kitchen to see if there are simple measures or equipment that can be put into place to make meal preparation easier.

Over the course of the program we revisit the goals, measure the results and recommend an action plan going forward.   Eat well and you can live well, stay active and feel great. 

Am I eligible?

Older people interested in healthy eating meal plans can apply for the Eat Well Live Well program under the Home Care Package (HCP) and Short-Term Restorative Care (STRC).

What’s involved?

The initial assessment takes an hour and a half and occurs in your home. Our qualified dietician will then return for a further five one-hour sessions with you as they implement a senior’s healthy meal plan tailored specifically to your dietary and nutritional needs.

Is there a cost for the program?

The  Eat Well Live Well program is available under the Home Care Package (HCP) and Short-Term Restorative Care (STRC) funding.  This means you will not pay for the cost of the program itself but may pay a contribution toward your care package.
