What Are Community-Based Supports?

Community-based supports are non-medical care programs and activities which are designed to improve the health and wellbeing of those people whose poor health is attributable to one or more of a range of social, economic and environmental factors. 

Community-based supports are aligned with the ‘Social Prescribing’ non-medical model of care which recognises that factors such as loneliness, isolation and social anxiety are common root causes of poor health in many individuals.
