What Is Case Management In Aged Care?

Case management is a service that supports senior Australians who are living at home and receiving a Home Care Package (HCP).
Case management helps to ensure that there are specific strategies in place to meet the individual health and wellbeing needs of the person receiving care. The role of the case manager is to design and administer a care management plan based on the
details in your ACAT assessment

If you are thinking of applying for a Home Care Package and would like to learn more about how plan management works, please contact Feros Care on 1300 090 256 and one of our Care Guides will be happy
