What is an NDIS plan?

Your NDIS plan outlines your goals and aspirations, and the supports you need to achieve these. It will be based on the discussion you had in your initial NDIS planning meeting. It will include the following information:

  • Your personal information 

Information about your disability, everyday activities, residential address and who you live with or whosupports you

  • Support from family and friends

Information about the support you receive from family and friends that helps you in achieving your goals but is not funded by the NDIS

  • Community groups and government services

Information about services and support funded and provided by community groups and government organisations

  • Your current and long-term goals

Information about your current goals (the goals you want to achieve with your NDIS plan) and your long-term goals (the goals you have identified for your life)

  • NDIS funded supports

Information about the funding that you have been allocated in each support category (Core Supports, Capacity Building & Capital Support).

Once your NDIS plan is completed, it will provide you with:

  • The services you can access under the NDIS

  • The amount of funding you will get for these services

  • The ability to choose the organisations and equipment you can spend your plan funding on.
