As The Healthy Me, Healthy Community program will be concluding service delivery as of the 30 September 2024, we will no longer be accepting program referrals.
Healthy Me, Healthy Community was launched as a pilot program with set timeframes. We now look forward to thoroughly evaluating the program and its outcomes to guide the development and future of the program and Social Prescribing in the region.
