Eat Well Live Well is designed to renew your interest in food and improve your appetite.  It will give you the energy you need to live a full and active life.

Over six sessions a dietician will work with you to help set goals and implement changes – all designed to improve meal times at home.

Nutrition advice, food monitoring, recipe ideas, tips and shortcuts, and even how to calorie load are just some of the topics the program covers.

We even take a peek into your pantry and kitchen to see if there are simple measures or equipment that can be put into place to make meal preparation easier.

Over the course of the program we revisit the goals, measure the results and recommend an action plan going forward.   Eat well and you can live well, stay active and feel great.

Am I eligible?

Older people can apply for the Eat Well Live Well program under the Home Care Package (HCP) and Short-Term Restorative Care (STRC).

What’s involved?

The initial assessment takes an hour and a half and occurs in your home.  Our qualified dietician will then return for a further five one hour sessions as they implement a program tailored specifically to your needs.

Is there a cost for the program?

The program is available under the Home Care Package (HCP) and Short-Term Restorative Care (STRC) funding.  This means you will not pay for the cost of the program itself but may pay a contribution toward your care package.
